If you qualify for a health insurance waiver, be sure to submit the waiver form in time for the next enrollment period.

A percentile premium reduction was be built into the 08-09 charge to offset this shift. Coverage runs continuously until August 9, 2008 for 1-3 year students while 4th year students are covered through June 30, 2008.
The entire medical coverage brochure, dependent application, as well as the waiver for those covered by a parent or a spouse, are available at Aetna Student Health website. To get to the web page Aetna has created for IU School of Medicine students, go to http://www.aetnastudenthealth.com. Click on Find your school and enter group number 812801. Click on Indiana University School of Medicine. Once there, you will see a link to the online waiver under the Enrollment and Waiver tab.
Students who are covered under the plan used by a parent or spouse that is effective while in Indiana, may file a waiver and receive a credit for the insurance fee. This must be the same plan used by the parent or spouse. Waivers for the full 2007-08 school year must be done on-line at the website and completed no later than September 15, 2007. Waivers for half-year must be filed manually no later than February 15, 2008. A new waiver must be filed every year. The on-line waiver is quick and easy and you will receive a confirmation printout as well as an email with a confirmation code upon completion. Credits will only be issued when we have received confirmation from Aetna that you have completed an acceptable waiver and you have been billed and assessed for the fall term, usually by mid-August. IUSM reserves the right to audit waivers for validity. The on-line waiver system exceeds the security requirements established by HIPAA. A complete description of Aetna’s Confidentiality Policy can be found at http://www.aetnastudenthealth.com.
All students will be automatically enrolled and assessed at registration, so there is nothing more to do in order for coverage to begin. However, if you are participating in the School's Plan, it is very important that you have your address information up to date with Christine Padgett at cepadget@iupui.edu. Aetna Student Health does not require you to complete an application to begin coverage, but will need an address in order to mail you statements, replacement cards and other information. Insurance identification cards will be mailed to the address you have on file with Medical Student Affairs after the September 15, 2007 waiver deadline has passed.
The cost for this Plan is $2,576 per year. Students will be billed by the IUPUI Office of the Bursar in two installments: $1,288 with the fall semester fees and $1,288 with the spring semester billing. Fourth year M.D. students who will be graduating in 2008 will have their coverage terminated on July 1, 2008 and will consequently only be assessed $950 with the Spring 2008 fees.
Coverage for eligible spouse and children may be purchased directly from Aetna Student Health by mailing in the application available at the website. Dependent applications must be renewed each year for coverage to continue. Applications will be available on-line soon and are due by September 15, 2007.
You may choose to enroll in one of two voluntary dental programs at an additional cost - the Aetna Advantage Dental Plan (a managed care dental plan) or Vital Savings by AetnaSM (a dental discount program.) Compare these dental plan benefits, networks, and costs in your area to see if one is right for you.
Due to a lack of sufficient dental network providers that cover the entire state of Indiana, Aetna Advantage dental providers may not be available in your area. If so, you may want to look into the Vital Savings discount program. The discount program will cost $25 per year for the single student. Please see the 07-08 brochure and dental link at the Aetna website for more information. Vital Savings applications will be available soon and there is no deadline.
Rates for the Aetna Advantage Dental Plan will cost $165.00 per year for the single student. Dental coverage runs from September 1, 2007 until August 31, 2008. The dental brochure and dental application can also be found at the website by mid-July. Remember, applications for both dependents and dental coverage must be resubmitted with Aetna Student Health each academic year. Advantage Dental applications will be available on-line soon and are due by October 1, 2007.
If you find yourself without medical coverage at any time during the year, you are required to prorate onto the School Plan on the day your waiver coverage lapses. Please contact John Keller for information regarding the prorated policy.
Remember that no plan covers 100 percent of medical costs. However, this Plan does cover most costs if a network provider is utilized. (Please be sure to view policy changes for 07-08 school year, most notably a new Rx deductible and dependent premium increases.) If you need to find a doctor or wish to check to see if your current doctor is in network, please go to http://www.aetnastudenthealth.com or at Aetna's DocFind Service at http://www.aetna.com/docfind. Make sure to look for doctors available under Open Choice PPO. Aetna Student Health's Customer Service Team can be contacted at 1-800-239-9691 if you need further assistance.
